1st step in communication

My 34 year old bother has severe CP and we have never really been able to communicate. I can only imagine the day that he can tell us "yes" or "no". To be honest, I'm not sure how much of what we say he understands. But it would be fantastic to be able to ask him questions and understand his response whether verbal or physical. Sometimes we just want to know if he is in pain or feeling hungry.

We have tried the images on his wheelchair tray but it didn't help. We feel that he has good control over his eyes so we tried the eye gaze boards, but the responses weren't consistent.

Can anyone help us?
